Change Of Details

If you change your name, address or telephone number, please let the practice know as soon as possible either by telephone, (01732 454545), or by calling in at reception during opening hours.

If you move outside of the practice area, you will be asked to register with a doctor in your new area.

The instructions regarding moving to another area are on your medical card.
