New Patient Registration
If you have recently moved into the area and would like to reigster with Town Medical Centre, please check that your address is within our practice catchment area.
We are unable to register you with the practice if you do not live within this catchement area.
If you do live within our the practice catchment area you can either complete the online registration form or collect a registration pack from reception.
All patients are now allocated a named doctor and you will be informed of who your doctor will be. You will also be invited to a New Patient Health Check.
As from immediate effect and until the 31st of December 2025, the practice is not accepting new patient registrations. We have taken this decision to ensure that the care of our existing patients is safe and not compromised and that we can safely meet all the demands of consultations with due regard to the well-being of our GP’s and staff as well as patients. We are able to do this on an informal basis under Regulation 171 of our GMS contract with Kent & Medway Integrated Commissioning Board. We will continue to accept new residents in Rockdale House.
Temporary Services
If you are unwell when you are away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. This registration should not be used if you need a repeat prescription. In this case you should contact your GP surgery who can then send an electronic prescription (EPS) to a pharmacy of your choice.
To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
Your Named GP
All Patients registered with the surgery will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the surgery. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
Proxy Access
A GP surgery can give someone proxy access so they can help another person manage their GP health and care.
A proxy may be able to act for the person they support, by:
- ordering repeat prescriptions
- booking appointments
- contacting the surgery or speaking to surgery staff
- viewing test results or vaccinations
- accessing all or part of the GP health record, to help with health-related tasks and managing health issues
