Online Prescription Ordering
You can request your prescription in 1 of 2 ways online:
1. Via Email -please email the practice with your name, date of birth and the medictions you would like to email@example.com
2. Via the Patient Access App
How do I use Patient Access?
Order online by clicking
You can ONLY use this service if you have already got an online account set up. If you do not have an online account for prescriptions, then you will need to:
Download the registration form or you can pick one up from the foyer of the building.
Bring this into Reception with 2 forms of identification (1 MUST be photographic and 1 MUST show your address) We will take copies of this and our secretaries will create an online account for you.
Paper Prescription Ordering
If you are unable to order online, you can write your request on any piece of paper or use a repeat prescription form and place this in the BLACK box on the stairwell. We check this box at least 3 times per day, please DO NOT come into the practice to deliver this prescription as we will not be able to take it.
Telephone Prescription Ordering
With regret, we are unable to take prescriptions over the telephone other than from those that are
Clinically Extremely Vulnerable (this changes therefore please accept guidance from the receptionists)
